for Sebastian’s Action Trust Santa Fun Run.

1.     I confirm that I have no health problems that might require medical attention during the event.

2.     I acknowledge that I am participating in the event at my own risk and that Sebastian's Action Trust cannot be held liable for any injury, loss or damage caused or sustained as a result of my participation.

3.     I realise that this is a fundraising event and pledge to raise sponsorship if possible.

4.     I realise that entry fees are non refundable as they cover the cost of administering the event.

5.     I understand that Sebastian's Action Trust may take photographs and video footage of you as part of the event and permit them to use the images for promotional purposes.

6.       I understand all persons under the age of 16 must be accompanied by an adult
