By entering and purchasing the ticket(s) you are agreeing to the following:

You are physically fit to compete

You willingly assume full responsibility for any and all risks that you exposing yourself to as a result of my participation in the fitness competition at FORM Leeds and accept full responsibility for any injury or death that may result from participation in said fitness competition. You hereby certify that you know of no medical problems that could increase my risk of illness or injury as a result.

Team Names at time of booking are final and we will not accept any changes. You can of course, change the name of any of the participants.
