By entering and purchasing the ticket(s) you are agreeing to the following:

You are physically fit to compete

You willingly assume full responsibility for any and all risks that you are exposing yourself to as a result of my participation in the fitness competition at FORM Leeds and accept full responsibility for any injury or death that may result from participation in said fitness competition. You hereby certify that you know of no medical problems that could increase my risk of illness or injury as a result.

We will never share your personal details with any third parties. 

Registration is open until Sunday 1st November and Team Members and Team Names can be changed up until this date.
