I represent and warrant that the information provided is correct and I am aware of no reason that would prevent my full participation in the audition provided by Fourth Monkey. I have read and understood the Audition Guidelines.

In line with the Fourth Monkey Audition & Offers Policy, I understand that if I am successful at audition, I will be required to complete a form with our accrediting partner Falmouth University to formally accept my offer to train with Fourth Monkey within the specified time-frame and will thereafter be aligned with the requirements of the Falmouth University Admissions Policy.
