Participation & Consent Declaration

By submitting this form I, the registrant and/ or participant, confirm that I have read the guidance provided on these pages - where a participant is under 18 I confirm that I have read these to the child and they understand these - and hereby agree that I will adhere to the session "Rules of Participation" as described here. 

I also confirm and/ or consent to the following:

The participant is in good health and considered capable of taking part in the programme. I have stated all relevant medical information above and consent that in the event of any illness/accident, basic first aid can be administered to the participant by trained staff. I also understand that while staff will take every precaution to ensure that accidents do not happen, they cannot necessarily be held responsible for any loss, damage or injury suffered by the participant.

Staff will only provide supervision and instruction at the venue during the session, and will not be responsible for anything that happens outside of this. Participants will respect the staff, facility and equipment and promote fair play at all time. Participants will avoid racist, sexist or abusive language or behaviour and there will be no violence, spitting, smoking, alcohol or drug consumption during or around sessions.

I agree that all of the information given in this form is accurate and I agree to the consent statements unless stated. If any of my/my child's details change I will update the instructor delivering the session accordingly.
